ABSTRACT

Objective: To explore the application of digital nursing management mode in the management of endoscope instruments of urological surgery. Methods: The digital nursing management mode was adopted to carry out management training for personnel of operation room, and improve management process for instrument of urological surgery, and fortify measurements of cleaning and maintaining for instruments. And the endoscope instrument error rate of pre-operation, missing parts number and doctors' satisfaction between pre and post implementation of digital nursing management mode were compared and analyzed. Results: The digital nursing management mode reduced the error rate of endoscopic instrument for department of urology surgery (x2=5.351, P<0.05), and reduced the number of missing parts (x2=4.033, P=0.05), and enhanced doctors' satisfaction(x2=67.944, P<0.01). Conclusion: Digital nursing management mode can strengthen the standardized management for endoscopic instrument of urology surgery, and reduce the error rate of endoscope instrument pre operation and number of missing parts, and effectively enhance the doctors' satisfaction. Therefore, it is worthy to be popularized and applied.

ABSTRACT

OBJECTIVE:To study the effect of EGCG-Zn chelated by epigallocatechin gallate (EGCG) and Zn ion on learn-ing,memory and antioxidant abilities in vascular dementia(VD)rats. METHODS:Rats were randomly divided into sham group, model group,zinc gluconate group (positive control,70 mg/kg),EGCG group (5 mg/kg),and EGCG-Zn low-dose,mid-dle-dose,high-dose groups (2,5,10 mg/kg),10 in each group. The VD models were induced by middle cerebral artery occlu-sion. The rats were intragastrically given relevant medicines after 7 d of modeling,once a day,for 28 d. Morris water maze were used to evaluate learning and memory abilities of rats. Spectrophotometric method was adopted to detect the superoxide dismutase (SOD),catalase(CAT),glutathione peroxidase(GSH-Px)activities and malondialdehyde(MDA)content in brain tissue of rats. RESULTS:Compared with sham group,escape latency was extended in model group,percentage of platform quadrant swimming distance(dP/dT)was reduced(P<0.01);SOD,CAT,GSH-Px activities in brain tissue were reduced,and MDA content was in-creased (P<0.01). Compared with model group,escape latency on 4th d of recording was obviously shortened in EGCG group and EGCG-Zn dose groups,dP/dT was obviously increased and dose-depended(P<0.05 or P<0.01);SOD,CAT,GSH-Px activi-ties in brain tissue were increased,MDA content was decreased(P<0.05 or P<0.01)and dose-depended. Compared with EGCG group and zinc gluconate group,escape latency was obviously shortened in EGCG-Zn dose groups,dP/dT was increased;SOD, CAT,GSH-Px activities in brain tissue were increased,MDA content was decreased;and the effects in high-dose,medium-dose groups were more obvious(P<0.05 or P<0.01). CONCLUSIONS:EGCG-Zn can improve the learning,memory and antioxidant abilities in VD rats,and the effect is superior to EGCG.

ABSTRACT

Objective:To develop an HPLC-DAD method for the simultaneous determination of seven alkaloid constitutents, so-phoranholN-oxide, oxymatrine, sophoridine, oxysophocarpine, sophoranol, matrine and sophocarpine in Sophora flavescens Ait. Meth-ods:The chromatographic separation was performed on an Agilent Zorbax C18 column (150 mm × 4. 6 mm, 5μm) with 10 mmol·L-1 NH4Ac(0.1% ammonia, pH=9.0)(A)-acetonitrile-methanol(1 ∶22) containing 20 mmol·L-1NH4Ac(B) as the mobile phase with gradient elution at the flow rate of 1 ml·min-1 . The detection wavelength was set at 220 nm,and the column temperature was maintained at 30 ℃. Results: SophoranholN-oxide, oxymatrine, sophoridine, oxysoph-ocarpine, sophoranol, matrine and sopho-carpine was linear within the range of 0.093-1.860 μg(r =0.999 6) , 0.530-10.600 μg (r =0.999 7), 0.062-1.240 μg (r =0. 999 8), 0. 281-5. 620 μg (r=0. 999 9), 0. 026-0. 520 μg (r=0. 999 8) ,0. 036-0. 720 μg (r=0. 999 7) and 0. 032-0. 640 μg (r=0. 999 6), respectively. The average recovery was 97. 5%, 98. 2%, 99. 0%, 99. 4%, 99. 2%, 98. 2% and 98. 7%, and RSD was 1. 18%, 0. 92%, 1. 43%, 1. 04%, 0. 81%, 0. 43% and 0. 88%(n =6), respectively. Conclusion:The method is convenient, accurate and reproducible in the quality control of multicomponents in Sophora flavescens Ait.

ABSTRACT

Objective To study the protective effect of nalmefene hydrochloride on lung ischemia-reperfusion injury and its mechanism.Methods 40 rats were randomly divided into model group,high dose of nalmefene group,low dose nalmefene group and sham operation group equally(n =10).The lung ischemia-reperfusion model was established by occlusion of the left pulmonary hilum.The intravenous injection of nalmefene (20,10 μg·kg-1) was applied at 10 minutes before occlusion of the left pulmonary hilum in the high dose of nalmefene group and the low dose of nalmefene group,respectively.The sham operation group without occlusion of the left pulmonary hilum was not given any treatment.At 2 h after reperfusion,all rats were detected arterial blood gas value and then sacrificed.The specimens from the upper lobe of the left lung tissue were preserved to observe pulmonary lesions,detect the ratio of wet / dry weight and the expressions of β-endorphin and interleukin(IL)-17.Results Compared with the model group,the value of PCO2,the degree of pulmonary lesions,the ratio of wet / dry weight and the expressions of β-endorphin and IL-17 in lung tissue were significantly decreased (P < 0.01),while the value of PO2 was significantly increased (P < 0.01) in the low dose of nalmefene group.Compared with the low dose of nalmefene group,thevalue of PCO2,the degree of pulmonary lesions,the ratio of wet/dry weight and the expressions of β-endorphin and IL-17 in lung tissue were significantly decreased (P < 0.01),while the value of PO2was significantly increased (P < 0.01) in the high dose of nalmefene group.Conclusion Nalmefene hydrochloride may prevent lung ischemia-reperfusion injury in a dose dependent manner by reducing the production of β-endorphin and inhibiting the expression of IL-17 in lung tissue.

ABSTRACT

OBJECTIVE:To study the effects of carbamylated erythropoietin(CEPO)on cardiovascular microcirculation in rats with diabetes mellitus. METHODS:Rats were randomly divided into blank control group,model group,CEPO low-dose,medi-um-dose and high-dose groups(500,1 000,2 000 u/kg)with 12 in each group. The rats in the last 4 groups were reduced diabetes mellitus model. All rats were given relevant medicine intragastrically twice a week,coronary microcirculation endothelial cells were separated after consecutive 4 weeks. Enzyme-linked immunosorbent assay was conducted to detect levels of peripheral serum prosta-cyclin (PGI2),vasoconstrictor endothelin-1 (ET-1),angiotensin Ⅱ(AngⅡ) and von Willebrand factor (vWF) of rats in each group;in vitro CCK 8 test was used to detect endothelial cell activity(OD value);real-time quantitative polymerase chain reaction was adopted to detect proliferation-related genes(Ki67,p16),poptosis-related genes(Bad,Bax),and expressions of protein vas-cular endothelial growth factor(VEGF)and AngⅠ. RESULTS:Compared with blank control group,levels of PGI2,ET-1,AngⅡand vWF in serum in model group increased;OD value deceased;Ki67,p16,Bax and VEGF expression decreased;the difference was statistically significant (P0.05). CONCLUSIONS:CEPO maybe improve the coronary microcirculation function by upregulating VEGF expression in coro-nary microcirculation endothelial cells and promoting endothelial cells’regeneration.

ABSTRACT

In search of effective anticancer agents, fifteen new phosphonate derivatives were designed and synthesized. Their structures were clearly established by elemental analysis, IR, 1H NMR and 13C NMR, and their antitumor activities were evaluated by MTT assay. Preliminary results in bioactivity tests indicated that some title compounds exhibited better activity. Among the active compounds, compounds 4e, 4n had better inhibition effect on A-549 cells growth with IC50 values of 8.7±0.8, 8.2±1.0 μmol·L-1, the IC50 values of compound 4c was 9.8±0.9 μmol·L-1 against SGC-7901 cells and compounds 4l, 4n exhibited more potent activities against EC-109 with IC50 values of 9.5±0.6, 9.4±0.5 μmol·L-1.

ABSTRACT

Objective To explore the effect of multifunctional rehabilitation wheelchair on early rehabilitation of elderly patients after anesthesia recovery with hip fracture surgery.Methods Toally 34 elderly inpatients with hip fracture surgery treated with regular function exerciser between November 2013 to October 2014 were assigned as the control group and another 34 elderly inpatients with hip fracture surgery during November 2014 and November 2015 as the observation group.The control group was treated with routine nursing and early on-bed rehabilitation exercise and the latter with early off-bed exercises on a muhifunctional wheelchair.The two groups were compared in terms of hip function and daily life activities ability before surgery and 14 d after operation.Result The Harris hip function cores and activities of daily living scores of the observation group after surgery were significantly higher than those of the control group (P <0.05).Conclusion Multifunctional rehabilitation wheelchair can be effective in the improvement of the hip function and daily life activities for the elderly patients with hip fracture surgery.

ABSTRACT

Objective To optimize the volatile oil extraction and inclusion process of Wenweiyang capsules. Methods An orthogonal test was adopted in this study. The extraction technology was optimized for the yield of volatile oil regarding the amount of water loaded, grain size of medicinal material, and decoction time as factors. The inclusion technology was optimized for the inclusion yield and volatile oil inclusion rate using the ratio ofβ-CD:oil, amount of water and grinding time as factors. Results The optimized extraction parameters were as follows:breaking medicinal material through 10 mesh screen, adding 6 fold volume of water and extracting for 5 h. The optimized inclusion progress was grinding at theβ-CD:oil ratio of 81, loading equivalent amount of water and grinding for 30 minutes. The average yield of volatile oil is 1. 72%, the average inclusion rate is 93. 01% and the average volatile oil inclusion rate is 74. 82%. Conclusion The extraction and inclusion technology is simple, reliable, which can effectively retain the volatile oil and provide evidence for the preparation of Wenweiyang capsules.

ABSTRACT

AIM:To observe the pathological changes of the lower segment of nasolacrimal duct mucosa in rabbits at different stages after retrograde lachrymal dilated drainage tube implantation. METHODS:Totally 14 New Zealand rabbits were used in the present study. One side of nasolacrimal duct was obstructed to produce an experimental model and operated the reverse implantation of nasolacrimal duct intubation. Histological changes of the lower segment of nasolacrimal duct mucosa were observed by routine light microscope at 2, 4, 6, 8, 10, 12 and 14wk after the operation. RESULTS: Compared with the control side, the group of 2 and 4wk after surgery presented the inflammatory cytokine. The group of 12wk after the operation presented isolated granuloma. Group 12 and 14wk presented scattered granuloma. The size of the granulomas was smaller and the density of epithelioid cell and fibroblast were lower in group 12wk than those in group 14wk by HE and Masson trichrome stain. CONCLUSION:Recurrent Silicone Tube is used to treat nasolacrymal duct obstruction. Nasolacrimal duct can be narrowed and blocked again by granuloma, progressive fibrosis and adhesion of surrounding tissues when tube is in the duct more than 12wk.

ABSTRACT

<p><b>OBJECTIVE</b>To develop a rapid multi-residue assay for detecting 16 demanded by the European Union (EU).</p><p><b>METHODS</b>A recombinant penicillin-binding protein (PBP) 2x* from Streptococcus pneumoniae R6 was expressed in vitro and six β-lactams were conjugated to HRP by four methods. A rapid multi-residue assay for β-lactams was established with PBP2x* and HRP-conjugate.</p><p><b>RESULTS</b>PBP2x* was expressed and purified successfully and the ideal HRP-conjugate was identified. The multi-residue assay was developed. After optimization, penicillin G, ampicillin, amoxicillin, cloxacillin, dicloxacillin, oxacillin, nafcillin, cephalexin, ceftiofur, cefalonium, cefquinome, cefazolin, cefoperazone, cephacetrile, and cephapirin can be detected at levels below MRL in milk with simple pretreatment.</p><p><b>CONCLUSION</b>This assay developed can detect all 16 β-lactams demanded by the European Union (EU). The whole procedure takes only 45 min and can detect 42 samples and the standards with duplicate analysis.</p>

ABSTRACT

Objective To propose and numerically simulate a novel stent with triangular wire cross-section so as to compare the hemodynamic effect of endovascular stents in aneurysm with circular, rectangular and triangular cross section, respectively. Methods Three aneurysm models were constructed by implanting 3 kinds of stents separately, including one with bare circular wire cross-section stent (named CM), and one with bare rectangular wire cross-section stent (named RM), and one with bare triangular cross-section stent (named TM). An unstented aneurysm model was also constructed to serve as the control (named UM). Four models were preformed with numerical simulation of fluid-structure interaction under the same boundary conditions using finite element method. Results Compared with TM and CM, RM showed lower velocity, higher flow resistance and longer turn over time, which demonstrated that the effect of blood flow pattern on three stents was decreased in the order of RM, TM, CM. RM could reduce the magnitude and fluctuation of wall shear stress (WSS) more significantly than that of TM and CM. However, the pressure rise of TM and CM was lower than that of RM. WSS distribution and deformation results indicated the possibility of aneurysm development along the distal wall was higher than that along the proximal wall, and the top of aneurysm was in the highest risk of rupture. Conclusions Three kinds of stents show differences of hemodynamics in aneurysm treatment, which provides significant references for the structural design and optimization of endovascular stent.

ABSTRACT

Background Glaucoma is primarily characterized by the damage of retinal ganglion cells.The macular ganglion cell complex (GCC)thickness can be quantitatively measured using spectral domain optical coherence tomography(SD-OCT). Objective This clinical study was to explore the macular GCC thickness change in primary open-angle glaucoma (POAG) patient with SD-OCT. Methods A serial case-controlled study was designed.A total 101 eyes of 101 POAG patients and 41 normal eyes of 41 age- and refract power-matched normal subjects were cnrolled in the study.POAG patients were assigned to normal perimetry POAG group,early stage POAG group,advanced POAG group and late stage POAG group.Average macular GCC thickness(GCC-Avg),superior GCC thickness(GCC-Sup) and inferior GCC thickness (GCC-Inf)of subjects were measured by SD-OCT and compared among POAG patients and normal controls.Peripapillary retinal nerve fiber layer(RNFL) thickness was measured with time domain OCT(TD-OCT).The correlation between GCC thickness with RNFL thickness or mean deviation(MD) of perimetry were evaluated and analyzed.Informed consent was obtained from each patient prior to entering this study.Results GCC-Avg thickness,GCC-Sup thickness and GCC-Inf thickness were significantly decreased in the normalperimetry POAG group and early stage POAG group compared with the normal control group (GCC-Avg:t =5.411,10.247,P < 0.01 ; GCC-Sup:t =6.171,9.484,P< 0.01 ; GCC-Inf:t =5.281,8.592,P < 0.01 ).Also,GCC-Avg thickness,GCC-Sup thickness and GCC-Inf thickness were significantly decreased in the advanced POAG group compared with the early stage POAG group ( GCC-Avg:t =4.246,P<0.01 ; GCC-Sup:t - 2.419,P - 0.019 ; GCC-Inf:t =4.636,P<0.01 ),and GCC-Avg thickness,GCC-Sup thickness and GCC-Inf thickness were significantly decreased in the late stage POAG group compared with the advanced POAG group (GCC-Avg:t=2.095,P=0.040;GCC-Sup:t=2.756,P<0.01:GCC-Inf:t =2.018,P =0.040 ).The positive correlations were seen between GCC-Avg thickness,GCC-Sup thickness,GCC-Inf thickness and RNFL-Avg thickness,RNFL-Sup thickness,RNFL-Inf thickness respectively( r =0.802,0.825,0.856,P < 0.01 ).MD value of perimetry was positive correlated with GCC-Avg thickness in POAG patients ( r =0.601,P < 0.01 ). Conclusions SD-OCT can quantitatively measure and differentiate the GCC thickness in POAG patients.The GCC thickness gradually decreases with the development of POAG.There exist a well correlation between visual field defect and RNFL thinning.

ABSTRACT

Background The main cause of filtering surgery failure is over proliferation of fibroblasts in filtering channels,leading to excessive fibrosis and scar formation.Researches determined that p38 mitogen-activated protein kinase(MAPK) signaling pathway plays an important role in fibroblast phenotype transition. Objective The present study was to investigate the inhibitory effect of p38 MAPK inhibitor on myofibroblasts transdifferentiation and the extracellular matrix synthesis after filtration surgery in rabbit eyes. Methods Trabeculectomy was performed on 24 eyes of 12 clean New Zealand white rabbits to establish the filtering operative models.The models were randomized into model group,SB203580 group and mitomycin C ( MMC ) group.1 ml SB203580 ( 0.2 g/L) was conjunctively injected at the end of operation in the rabbits of the SB203580 group,and the cotton piece with 0.2 g/L MMC solution was placed on the operative area for 3 minutes intraoperatively in the rabbits of the MMC group.The bleb appearances were examined under the slit lamp microscope,and intraocular pressure(IOP) was measured with Icare tonometer I,3,7,10,14 days after operation.0.2 ml aqueous humor was extracted and the conjunctive tissue at the filtering area was obtained 14 days after operation for the detection of α-smooth muscle actin (α-SMA) and fibronectin protein by ELISA.Expression of ACTA2 mRNA,connective tissue growth factor(CTGF) mRNA and alpha2 chain of type Ⅰ collagen( COL1A2 )mRNA in conjunctive tissue was assayed with fluorescence real-time PCR. Results Vascularization of fibrosis of filtering bleb were obvious in the eyes of the model group,and the bleb was flat and diffuse in the eyes of the SB203580 group and MMC group on 14 days following operation.No significant difference was seen in IOP before trabeculectomy among these three groups( F=0.065,P=0.937 ).IOP was gradually elevated with the increase of time after operation ( F =32.873,P =0.030 ).ELISA assay showed that α-SMA level in conjunctiva was lower in the SB203580 group and MMC group compared with the model group,and that of MMC group was significant lower than the SB203580 group( P<0.05 ).Fibronectin level in conjunctiva was lower in the SB203580 group and MMC group compared with the model group,and that of MMC group was significant lower than the SB203580 group (P<0.05).Fluorescence real-time PCR showed that expressions of the ACTA2 mRNA,CTGF mRNA and COL1A2 mRNA were significantly different among the three groups( P<0.01 ),with the highest expression in model group and the lowest expression in the MMC group ( P < 0.05 ). Conclusions Fibrotic reaction after trabeculectomy can be suppressed by inhibiting p38 MAPK signal pathway.The mechanism of SB203580 is to reduce the synthesis of myofibroblasts transdiffercntiation and extracellular matrix.

ABSTRACT

Objective To reduce the hypokalemia caused by inner environment imbalance effectively in order to lower the death rate and improve the prognosis for patients with severe cranial injury.Methods Seventy-five patients with severe cranial injury who had hypokalemia were studied retrospoctively from October 10th,2003 to July 30th,2006.Results The waves of electrocardiogram and changes of illness condition was closely observed,the volume:of intake and output and kalium outcome was accurately recorded.Kalium was supplemented as early as possible.the amount was adjusted by the use of dehydratins agents and biochemical kalium.Only timely and effective monitoring and treatment could complications be reduced or prevented.Conclusions Severe cranial injury is one of the most severe trauma,timely monitoring and nursing measures are key point for rectification of hypokalemia for patients with severe cranial injury.
